How to submit your hair strand test sample for nutrient and toxin evaluation

1.Using a pair of scissors, straight edge, safety or electric trimmer cut the hair strands at the skin or scalp level.

 2.Collect a group of hair strands, full length about a 1/4" in diameter if held between your finger and thumb from the scalp. Roots or follicles are not required.

3. Place the strand sample in a clean, white, mailing envelope with the sample donors name, address, phone number, and email printed on the front.

4  Print out and complete the personal health questionnaire.

5. Place the envelope containing the hair sample, the completed questionnaire, and a check or money order ($375.00) payable to The Carlson Company LLC or a completed credit card information form

   in a second envelope addressed to;
